Care Manager II (HIV, Home Health and Behavioral Services)
The Care Manager II is responsible for independently enrolling a minimum of 3 cases per month (36 cases a year); Provide follow-up services to meet the client's needs and tracking for their caseload. Case load volume will be determined by the Administration of HIV, Health Home and Behavioral Services, adjustments to case load will be made according to agency need. Must use discretion and independent judgment in exercising duties.
