Transitional Care Liaison

The Senior Living Transitional Care Liaison serves as a strategic relationship builder supporting occupancy growth, resident retention, and continuity of care across Reliant's Living Rehab Senior Living and Reliant's Skilled Nursing Facility partner network. This role facilitates collaboration between Executive Directors, Directors of Rehabilitation, sales and marketing leaders, and other community stakeholders to strengthen referral relationships and create opportunities for resident transitions across the continuum of care.
