Quality Coordinator - Quality Assurance

The (QC) at Desert Valley Medical Group (DVMG) works closely with the Quality Manager, Medical Director and Administrator in implementing quality platforms. This includes chronic care management (CCM), transitional care management (TCM) and other quality initiatives including HEDIS, P4P, MIPS, STAR measures, and annual wellness visits. The QC assesses missing patient benchmarks and organizes the data in preparation for member outreach or encounter with healthcare provider. Through a combination of mailing patient due letters or education flyers, calling the patient and/or healthcare provider, and creating referrals the QC is instrumental in-patient satisfaction and quality outcomes.
