Consultant Pharmacist

Presbyterian Homes & Services is seeking a Consultant Pharmacist to join its Radius Living Rx team. The Consultant Pharmacist collaborates directly with the Director of Clinical Services and Radius client facilities to enhance patient health by ensuring optimized medication regimens. This role completes drug regimen reviews, provides subject matter expertise to facilities, patients, and families, identifies and addresses areas for improvement, and assists facilities with compliance with applicable laws and regulations.
