Social Worker - Care Management II

The Social Worker - Care Management II conducts comprehensive psychosocial assessments, provides counseling and support, and coordinates care plans to address the social, emotional, and resource needs of patients and families. This role collaborates with interdisciplinary teams to facilitate safe transitions of care, advocates for patient needs, and connects patients to community resources.
