Social Worker-Crestwood Medical Center-FT-1st shift

Prepares the patient and/or patient's family to transition from the hospital to the next appropriate care setting based on their individual needs. Applies their understanding of medical conditions, insurance coverage and how physical/psychosocial may affect the patient once a patient leaves the medical setting to plan and initiate an individualized discharge plan. Provide clinical information to payers and/or post-acute providers in order to obtain authorizations for hospitalized patients.
