Patient Referral Coordinator (Float position within Chicago area)

In this role, you will serve as the primary link between Skilled Nursing Facilities (SNFs), patients, providers, and clinical teams, ensuring efficient communication and seamless care coordination. This position combines administrative support with relationship management and requires regular on-site engagement with partner facilities throughout the Chicago area. This is a dynamic role within a growing market. Responsibilities, priorities, and scope may evolve as business needs change and new opportunities emerge. Regular travel and on-site visits (2-3 days per week) are required.
