What You’ll Do

• Conduct Comprehensive Psychosocial Assessments Assess the emotional, social, and environmental needs of patients and families, including caregiver stress, family dynamics, coping abilities, and safety concerns.

• Provide Emotional Support Counseling Offer compassionate counseling to patients and families navigating serious illness, grief, and end-of-life transitions.

• Collaborate with the Interdisciplinary Team Work closely with nurses, physicians, chaplains, and other care team members to develop and support individualized plans of care.

• Connect Families to Community Resources Serve as a liaison to community agencies and support services to ensure patients and families have access to appropriate resources.

• Bereavement Support Participate in and support hospice bereavement services, providing follow-up and resources to families after the loss of a loved one.

• Ensure Compliance Quality Care Maintain accurate documentation and ensure services align with hospice regulations, policies, and standards of care.

What We’re Looking For

• Master’s Degree in Social Work (MSW)

• Hospice or healthcare experience preferred (geriatrics or end-of-life care a plus)

• Strong interpersonal and communication skills

• Compassionate, patient-centered approach

• Ability to work independently while collaborating within a multidisciplinary team

• Cultural sensitivity and respect for diverse backgrounds and belief systems
