Special Projects Lead, Strategic Clinical Programs
Our Special Projects Leads take on big, ambiguous problems and own them end to end. This one owns our strategic clinical programs: focused offerings that give health systems something specific and proven instead of generic home health. These programs are what our partnerships are built on.
Ortho is live with its first partner. Neuro is being stood up. Cardiovascular and oncology are likely next, based on what partners ask for. You'll design each program, launch it in market, deliver what Partnerships sells, and produce the outcomes data that wins the next partnership. You'll work with Partnerships, Clinical, Operations, and market GMs, and you'll be in the room with hospital leadership. Like all SPL roles, this is a track toward owning a function, a P&L, or a GM seat.
Location Eastern or Central time zone preferred. Travel to New York and Texas 1–2x a month, more during onboarding.
Compensation $160k–$190k base plus performance bonus, with on-target earnings of $200k–$250k. Medical, dental, and 401(k) matching.
About Us We are fixing US healthcare by building an AI-native physical care platform, starting with home health. Home health is a $140B industry with less than $10B in EBITDA, and 40% of revenue goes to administrative waste. We automate that work with AI, which gives us a different cost structure from incumbents and lets us serve the 30% of patients who go untreated today. Your First Priority Our ortho program was built by hand around one partnership, and the outcomes are strong. Your first job is to make it run in any market: written protocols, clear staffing, reporting that doesn't depend on manual tracking, and a launch plan for the next partner.
What You Will Do
1. Design the program menu. Define each program: target patients, visit cadence, protocols, staffing, telehealth, reporting, and what we promise a partner. Decide which partner requests become programs and which don't. A few programs delivered exceptionally beat a long list delivered adequately.
2. Deliver what Partnerships sells. When we sign a health system, you own execution, build that partner's program and live reporting with our clinical leaders in market, and own the numbers we report back, including in at-risk structures where our payment depends on outcomes.
3. Scale across markets. Launch programs with Growth and Operations as we open new regions, and equip market GMs and RVPs to sell and deliver them.
4. Own the metrics. Set each program's KPIs (readmissions, time to start of care, patient outcomes, margin, referral conversion), know how each baseline was set and where it can be gamed, and build your own dashboards on our internal AI and data platform.
Must-Have Experience
- A clinical background: RN, PT, NP, PA, MD, or DO. You need credibility with clinicians and hospital clinical leadership.
- 3–5 years owning complex, ambiguous projects with many stakeholders and no playbook. You can explain the key decisions you made and why they mattered.
- Analytical rigor. You can use SQL, Excel, or AI tools like Claude Code to build a business case, set a baseline, and defend a number to a partner who wants to argue with it. This is not optional.
- Clear communication. You can turn a complex clinical and financial picture into a deck a founder would put in front of a health system executive.
- A track record of owning outcomes: work that measurably moved patient outcomes, volume, cost, or revenue.
How You Work
- You take a vague ask and return something excellent, and you run a rollout like a workflow with owners, deadlines, and failure modes.
- You get things done with clinical and operations teams without authority over them.
- You can discuss heart failure readmissions and the margin on the contract that pays for them in the same conversation.
- Low ego, high standards. You hold the line on what we are willing to promise.
Nice-to-Haves
- Built or ran a service line (ortho, neuro, cardiac, oncology) at a health system, or stood up a clinical program at a provider
- Home health, post-acute, or hospital-at-home experience
- Value-based care, at-risk contracting, bundled payments, or CMS models such as TEAM
- Health system partnerships or JVs
- MBA or management consulting background
